Eligibility will be as prescribed by NEET UG Brochure 2025 by State CET Cell, Mumbai, and as per eligibility from Admissions Regulating Authority (ARA), Mumbai & Maharashtra University of Health Sciences (MUHS), Nashik time to time. So, likewise Authorities reserves the right to make any changes in the Eligibility criteria.

List of Documents required at the time of UG (BAMS) Admission: Mandatory Documents (matching with uploaded documents at the time of registration)

1. Age, Nationality & Domicile
2. SSC Passing Certificate
3. HSC Mark sheet
4. NEET Mark sheet
5. Aadhar Card (Xerox Copy) & in original
6. Leaving/Transfer Certificate
7. Medical Certificate (on Registered Medical Practitioners letterhead) – marked for Ayurveda course.
8. Income Certificate (2024 – 25) (in case of students having family income below 8 lakhs) / Parents ITR.
9. CET Cell Registration Form for all Quotas.
10. NEET Admit Card (with photograph, thumb and duly signed by candidate and parent)
11. Passport size 05 Photos with white background (Hard copy & soft copy)
12. Undertaking in prescribed format regarding Fee Revision – must be handwritten & duly signed (Click to Download)
13. For All India Quota (AIQ) student require caste validity certificate, Non-Creamy Layer certificate should be in AACCC format (Govt. of India).
14. Hill area - Parents document only.

Documents as per applicability:

1. Caste Certificate (bearing number matching with Caste Validity certificate) as per applied quota & in prescribed format for AIQ/ State – Institutional Quota (Refer NEET 2025 & AACCC 2025 brochure)
2. Caste Validity Certificate (bearing number matching with caste certificate)
3. Non-Creamy Layer Certificate (valid upto 31.03.2026) as per applied quota & in prescribed format for AIQ/ State – Institutional Quota (Refer NEET 2025 & AACCC 2025 brochure)
4. Migration Certificate
5. Gap certificate
6. If claimed Hilly Area, Defence 1/2/3, Minority etc – documents to be produced as notified in NEET 2025 & AACCC 2025 brochure
7. Notarized undertaking in prescribed format on Rs 100 bond paper regarding fee revision (Click to Download); If student retains the admission in the institute.

Important Note:

1. Candidate has to produce all the above said original documents for verification at the time of admission, must be matching with previously uploaded documents at the time of registration.
2. Candidate should bring 03 Xerox Sets of above documents (without attestation) at the time of admission.

3. Any photo ID Proof uploaded at the time of registration (Aadhar Card/Pan card) (Xerox Copy) & in original
4. AIAPGET 2025 Mark sheet
5. Age, Nationality & Domicile
6. SSC Passing Certificate
7. BAMS Marksheets
8. BAMS Degree Certificate/Passing Certificate
9. Internship Completion certificate
10. Leaving/Transfer Certificate
11. MCIM Permanent Registration Certificate/ Application and fees receipt for Permanent Registration to MCIM
12. Medical Certificate (on Registered Medical Practitioners letterhead)
13. Passport size 05 Photos with white background (Hard copy & Soft copy)

Please Note: In case of loss of document no. 6 & 7 – Affidavit, FIR and Duplicate copy of the document from concerned authority is mandatory.
